Electronic Questionnaire Templates

Electronic questionnaire templates have been made available for all surveys conducted by the C&SD that are amenable to the electronic mode of delivery. Survey respondents are encouraged to complete their survey questionnaires in electronic format and return them by electronic means.

Upon request by survey respondents, electronic questionnaire templates are delivered to survey respondents via email or during field visit and they can return the completed templates to C&SD by email (if returning the template by email, the respondent is advised to apply encryption with the use of e-certificate¹, for better protection of confidentiality of the supplied data) or ask C&SD to collect the completed templates during field visit.

The availability of electronic questionnaire templates together with the Public Key Infrastructure² (PKI) provides a more convenient, faster and safer method for supplying data to C&SD.
