This form is prescribed by the Privacy Commissioner for Personal Data for making data access request under the Personal Data (Privacy) Ordinance.

Enquiries related to employment-related personal data can be addressed to (852) 2582 3038, while enquiries related to non-employment-related personal data can be addressed to (852) 2582 4731.
