This is the form for acquiring statistics from the Census and Statistics Department (C&SD) via facsimile transmission. The service is available to local customers only. To use the service, a customer is required to open an account in the C&SD using Form 407. Form 407 can also be used by customers for changing particulars of their accounts or putting in further deposits in their accounts. To request for statistics after opening an account, a customer can just fill in Form 408 and fax it to the C&SD. Please refer to the guidance notes for details.
